This web site is really a modified version of PFSurvey.
PFSurvey is a web-based application used to collect survey/polling information. Designed as a rapid prototype, a web-based survey can be implemented in a few minutes.
It's perfect for I/T related feedback on apps or Organizational feedback.
Information is collected and stored using an Access 97 database.
With PFSurvey, there is no need for mailing out survey forms and manually entering information, and therefore can be a real time saver.
Included with the application is a dynamically linked Excel workbook containing the survey, all survey responses, with summaries of the aggregates, and charts so there is no need to spend time summarizing the survey data.
The Survey can be ANONYMOUS , or can capture information about user for further analysis.
